Prescription Break-Even Method

Pay-per-item cost:

pay per item = items per month x months x prescription charge

3-month PPC comparison:

3-month saving = pay-per-item cost for 3 months - 3-month PPC cost

12-month PPC comparison:

12-month saving = annual pay-per-item cost - 12-month PPC cost

HRT PPC comparison:

HRT saving = listed HRT item cost - HRT PPC cost

The current standard NHS prescription charge in England is GBP 9.90 per item for 2026/27, with the standard PPC prices frozen at GBP 32.05 for 3 months and GBP 114.50 for 12 months. The page keeps these values editable because charges are reviewed, and because a local copy of this page may not be updated before the next charge year.

A 3-month PPC pays off when you would otherwise pay for 4 or more items in 3 months. A 12-month PPC pays off when you would otherwise pay for 12 or more items in a year. The HRT PPC is different: it only covers listed HRT medicines, so a person with an HRT item and a non-HRT item may need both an HRT PPC and normal charges or a standard PPC, depending on the mix.

Worked Prescription Cost Examples

Two Items Every Month

Two chargeable items per month would cost GBP 237.60 over 12 months at GBP 9.90 each. A 12-month PPC at GBP 114.50 would save GBP 123.10.

Four Items In Three Months

Four items in a three-month period cost GBP 39.60. A 3-month PPC at GBP 32.05 saves GBP 7.55 and removes item-count worry for that period.

One Listed HRT Item

One listed HRT item each month costs GBP 118.80 over a year if paid per item. An HRT PPC at GBP 19.80 can save much more, but it covers only listed HRT medicines.

Before Buying A PPC

Check whether you are exempt from prescription charges before buying a PPC. Exemption can apply because of age, pregnancy, certain benefits, a valid medical exemption certificate, war pension exemption, NHS tax credit exemption, low income scheme help or other rules. Incorrectly claiming free prescriptions can lead to penalty charges, but paying for a PPC when you are exempt can also waste money.

The calculator is for NHS prescriptions dispensed in England. Prescriptions are not charged in the same way in Scotland, Wales and Northern Ireland. Private prescriptions, dental charges, wigs, fabric supports and some appliances have different rules. A prescription charge is per item, not necessarily per paper slip. If a prescription has two different chargeable items, there may be two charges.

For regular medicines, compare the timing as well as the headline saving. A 12-month PPC can be paid in instalments by Direct Debit, but it still covers a fixed certificate period. Start it when the first chargeable prescription is due, not weeks earlier. For a short burst of medication, a 3-month PPC may be better than an annual certificate.

Planning Around Repeat Prescriptions

Repeat prescriptions can make PPC savings more likely, but the timing still matters. If a medicine is reviewed every two months, count the items actually dispensed during the certificate period rather than the number listed in a repeat template. If one medicine stops halfway through the year, the annual PPC may save less than expected. If a new medicine is added, a PPC can become worthwhile sooner.

For households helping an older relative, keep prescription charge planning separate from medicine safety. A PPC is a payment product, not a medication review. Questions about side effects, interactions, missing doses, delivery services or monitored dosage packs should go to a pharmacist, GP surgery or prescriber.

Certificate Date Checklist

Check start dates before paying. If the next prescription is not due for several weeks, buying a PPC today can waste part of the certificate period. If several items are due together next week, starting then may cover more real prescriptions. Keep the certificate confirmation, expiry date and payment plan with household medical paperwork so renewals are not missed. Add a reminder one month before expiry if regular medicines are likely to continue.

FAQs

How many items make a 3-month PPC worthwhile?

Four chargeable items in 3 months. Four items cost GBP 39.60 at GBP 9.90 each, which is more than a GBP 32.05 3-month PPC.

How many items make a 12-month PPC worthwhile?

Twelve chargeable items in a year. Twelve items cost GBP 118.80, which is above the GBP 114.50 12-month PPC.

Does a PPC cover all NHS prescription items?

A standard PPC covers NHS prescription charges in England during the certificate period. It does not cover private prescriptions or other NHS charges outside its scope.

Does the HRT PPC cover non-HRT medicines?

No. The HRT PPC covers listed HRT medicines only. If you also pay for other medicines, compare a standard PPC with HRT PPC plus item charges.

Should I buy a PPC if I am exempt?

No, not for cost reasons. Check exemption rules first. If you do not have to pay prescription charges, a PPC is usually unnecessary.

Are prescriptions free outside England?

Prescription charging differs across the UK. This break-even calculation is for NHS prescription charges in England.
